New Patient Check List
We're glad you've chosen us to care for your eyes. Before you come to your first appointment, here are some things you can do in advance to streamline your visit.
Step 1: New Patient Form
Fill out a New Patient Form. The more you can tell the doctor about your personal medical history, your activities, and any problems you have with your eyes, the better he can help you.
Step 2: Check Your Insurance
If you have a vision plan that you plan to use for your visit please inform our staff in advance so that we may obtain you authorization. Also, many medical plans (BCBS, United Health Care, Tricare, etc..) will cover the health portion of your eye exam, and we are participating providers with many of these plans. If you aren't sure, contact our office or your insurance provider.
Step 3: Don't Forget your Insurance Card
Without your Insurance Card, we will not be able to process your claim.
Step 4: Make sure you know where we are.
For new patients, our office can be hard to find. Here's a map to help you find your way. Remenber: our office is on the back side of the building!
